Superbills your patients can actually submit
Enter your practice once, record each visit with its codes and fees, and generate a print-ready superbill — provider identifiers, patient details, diagnoses, dates of service, coded line items, payments, and totals — that your patient hands to their health plan to request out-of-network reimbursement. All of one patient’s visits go on a single document.
100% client-side — patient names, diagnoses, and fee data never leave this browser. No uploads, no accounts, no servers. Nothing here is transmitted anywhere, so no patient data is ever stored by this site.
Not a billing service, and reimbursement is never guaranteed. This is a documentation aid. Whether a plan pays is decided by the patient’s own plan. Procedure and diagnosis codes change every year — confirm the codes you use against the current code sets and your payer’s guidance. Items marked verify need your own confirmation.
1 Practice & provider profile
Entered once — printed on every superbill. Payers expect the rendering provider’s name, NPI, and practice Tax ID (EIN); a superbill missing them is a common reason requests stall. verify
2 Patients
Add each out-of-network patient, then select one to record visits in Step 3. Member ID and group number are optional — include them when the patient gives them to you, since they help the plan match the record. verify

Diagnosis codes (ICD-10-CM)
Payers almost always require at least one diagnosis code. Enter the codes this patient’s chart supports, comma-separated. The quick-picks are common examples only — ICD-10-CM is revised every year, so confirm against the current code set. verify
Quick-pick examples:
Visits
Add a visit
Place of service follows the CMS code set (11 = office, 02 / 10 = telehealth); some payers direct a different code for telehealth — confirm with the plan. verify
Service lines

4 Generate & print the superbill PRO
Builds one document covering every visit recorded for the selected patient, with a completeness check first: missing elements a payer is likely to require are listed as warnings before you print.
5 Backup
Everything autosaves in this browser. Download a backup file before switching computers — patient data is sensitive, so keep backup files somewhere safe and encrypted.
FAQ
Does any patient data leave my browser?
No. Provider profile, patient names, diagnoses, and fee data are processed and stored only in this browser’s local storage. Nothing is uploaded to any server, so there is no third-party storage of patient information to manage. Your own obligations for safeguarding patient information (including on this device and in backup files) still apply — follow your practice’s privacy and security policies.
What is free and what is Pro?
Free: generate up to 3 superbill documents, with print for each. Pro ($14 one-time) unlocks unlimited superbill documents plus batch printing of every patient’s superbill in one go.
Will my patient definitely be reimbursed?
No — and no tool can promise that. The patient’s plan decides, based on its own out-of-network benefits, deductible, allowed amounts, and filing deadlines. This tool only produces a complete, legible statement with the elements payers ask for; it does not submit anything to any insurer.
Are the quick-pick codes the right codes for my patients?
The quick-pick CPT, ICD-10-CM, and place-of-service lists are small convenience samples, marked verify. CPT is revised annually by the AMA, ICD-10-CM every fiscal year by CMS/CDC, and place of service by CMS. Only record codes your own documentation supports and confirm them against the current code sets — this tool does not give coding advice.
Can one superbill cover a whole month of sessions?
Yes. Record each visit under the patient in Step 3 and the generated document lists every visit with its own lines, subtotals, and payments, plus one combined summary — the format most patients submit monthly. verify — a few plans ask for one statement per date of service; check the plan’s instructions if a request is denied.
Will my work be saved if I close the tab?
Yes — every change autosaves to this browser’s local storage. Use Step 5 to download a backup file if you work on more than one computer.
